Colorado Heart Healthy Solutions
The Wray Community District Hospital will be collaborating with Yuma District Hospital to bring an exciting opportunity to our community. The prospect of reducing the risk of developing heart disease by making some small changes in your lifestyle is offered through this program. Yuma District Hospital is providing this Colorado Heart Healthy Solutions program through grant funds from the Cancer, Cardiovascular Disease and Pulmonary Disease Grants Program at the Colorado Department of public Health and Environment.
Colorado Heart Healthy Solutions is a community of health workers, public health professionals and medical providers who help people like you live healthier lives in their communities. The community health workers (CHWs) test your blood pressure, cholesterol, blood sugar and weight at no cost to you. If you are looking for ways to improve your health, the CHWs can guide you to medical clinics and healthy living programs in your area such as walking clubs, diabetes management classes and nutrition programs. The CHWs can also support you for up to one year through phone calls and retest your blood pressure and cholesterol to check your progress.
Screening opportunities are being offered throughout the Wray and Idalia areas: Rhonda Metcalf will be at the Wray Hospital South Education Room on Wednesday, October 5th and every 1st Wednesday of the month to do screenings. Appointments are necessary and can be made by calling Rhonda at Yuma District Hospital 970 848-4705.
